Registration (Adults)

Registration is the first point of contact with the Mental Health Center. Designated Center staff are available in the Admissions/Crisis office to screen and evaluate persons for the first time, and to screen and refer those in crisis. If you are a resident of Madison County, Alabama, you may register in person, by phone, or through this web site. The Mental Health Center is located at 4040 Memorial Parkway SW. If you would like to pre-register online, you may do so by clicking here to open a document which you can fill out. Once you have completed the registration form, save it and email it to ranae.hicks@mhcmc.org as an attachment. Alternatively, you can print out the completed form and bring it with you when you register in person.
